考試委員名冊 (最遲於口試前半個月前通知學程辦公室)
List of dissertation committee members (Notify the program office at least half a month before the oral
examination)

學位考應試文件 (口試前前至學程辦公室領取)
Documents for the degree examination (Collect from the program office before the oral examination)

口試委員審定書 (自行列印)
Oral examination committee approval form (Self-printed)

學位考申請時間:上學期為11月30日前，下學期為4月30日前。
Application deadlines: Before November 30th for the first semester, and before April 30th for the second
semester.

如無法於該學期結束口試之同學，請記得繳交考試撤銷申請書。
If you are unable to complete the oral examination by the end of the semester, please remember to submit an
examination cancellation application.

請申請辦理畢業離校同學注意，在不同階段繳交下列表件，以順利完成離校手續。
Graduating students should pay attention to submitting the following items in different stages to complete the
departure procedure smoothly.

After successfully passing the oral examination, please submit the following documents to the department office:

口試委員口試費用請領冊
Receipt for oral examination committee fees

Thesis/Dissertation Academic Ethics and Originality Comparison Declaration
- 經由圖書館系統將論文上傳後繳交下列文件至系辦公室。
After uploading the dissertation via the library system, please submit the following documents to the department
office:

校圖書館平裝或精裝論文 2 本
Two copies of the dissertation, either in softcover or hardcover

指導教授同意該生至學程辦公室辦理離校手續申請單
Approval from the dissertation advisor for the student to apply for the departure procedure at the
program office
